Hospice Care

Hospice is care and support for terminally ill people and their loved ones. The Hospice philosophy encourages the continuation of a meaningful life in the presence of a life-limiting illness. Hospice neither prolongs life nor hastens death, Staff members assist in minimizing pain, offering comfort and helping patients maintain their dignity.

Crozer Health ’s team offers physical, spiritual and emotional support for patients in both hospital and in-home settings. Care can be provided in your home, in an assisted living center or personal care home, in a nursing home, or in our inpatient Hospice Residence located at Taylor Hospital.

How Hospice Care Enriches Lives

Bereavement Support Group

Are you grieving the recent loss of your loved one? Learn how to cope through education and support. Crozer Health offers free six-week sessions to all adults who have recently lost a loved one. Groups are facilitated by qualified and caring professionals.

Candidates for Hospice Care

A patient’s family member, friend, physician or other healthcare professional may request a consult. Acceptance is based on qualifying criteria and a physician’s order. The Crozer Health Hospice Care team will gladly work with anyone who is interested in these services to evaluate a patient’s needs and eligibility.

Guidelines and Policies

Intake specialists will make all of the necessary arrangements and perform a full evaluation of insurance coverage before a patient is admitted.
