Referrals

For your convenience, physicians and other clinicians can download the Sleep Centers Referral Form for patient referrals. Simply fill out the form and fax it to 610-340-2371.

Referring clinicians may choose to send patients to a Crozer Health Sleep Center for a direct study, with or without follow-up care with a sleep specialist:

For a directly ordered sleep study:

The ordering clinician must provide a pre-authorization for the study (in most cases, depending on the patient's insurance plan), patient demographics, insurance card copies and the patient's medical history.

It must also be indicated at that time whether the patient is to see a sleep specialist for follow-up or not. If not indicated, it is the ordering clinician’s responsibility to review study results with the patient and for any follow-up and/or treatment plan.

To refer a patient for a full evaluation:

The other option is to refer the patient to one of our sleep specialists for a full sleep evaluation, in which everything is handled by us - from the ordering of the appropriate study all the way through to complete treatment of the patient's sleep disorder(s).

Cancellation/Late Policy

Please Note: If you need to reach your sleep technician on the night of your study for cancellation or lateness, please call the following number for the appropriate location:

Crozer Health Pavilion at Brinton Lake: (610) 459-2479 (after 9:00 p.m. only)

Delaware County Memorial Hospital : (610) 394-4820 (after 9:00 p.m. only)

Taylor Hospital : (610) 595-6260 (after 9:00 p.m. only)

If for whatever reason you cannot attend the study, you must give at least 24 hours notice so that we may schedule another patient in your place.

Billing Information

Your invoice will consist of the following:**:

  1. Your physician will bill for the interpretation of the study.

You are responsible to bring your co-payment with you the night of the study in the form of a personal check or money order. Please do not bring cash. Please contact your insurance company to determine the correct dollar amount.

**Please check with your insurance company for sleep study coverage. Sleep studies are considered outpatient tests and will be billed as such. (Procedure code: PSG 95810 or C-PAP 95811, Diagnosis code: 780.53).
