Complex Health and Social Needs Fellowship
Formerly the Healthcare Hot-Spotting and Super-Utilizer Fellowship
As our nation and healthcare systems advance, we are challenged to create and implement innovative and sustainable approaches to expanding healthcare access while providing quality care. To achieve that goal, the Crozer Health and the Camden Coalition of Healthcare Providers have established a unique fellowship to address the disproportionate use of healthcare resources, especially by those patients who suffer from chronic and complex medical conditions.
In Camden, New Jersey the Camden Coalition of Healthcare Providers have used community-based, intensive care coordination to achieve great clinical advances for patients who frequently utilize area emergency rooms and hospitals. This yearlong fellowship seeks to train Family Physicians and Nurse Practitioners to become clinical champions for the development of strategies and the implementation of systems to reduce over-utilization the efforts of integrated, inter-professional teams at the Camden Coalition and Crozer Health .
About the Program

Psychologists in integrated health care: Super-utilizer team care
Our one year fellowship brings together collaborative, integrated clinical teams including Family Physicians and Nurse Practitioners, as well as social work and psychology students and pharmacy residents, to become clinical champions for the development of strategies and the implementation of systems to address the issue of frequent acute care utilization by working closely with the Camden Coalition and Crozer Health. Fellows possess a keen interest in working with underserved populations and developing skills in healthcare hot-spotting, healthcare policy, a patient-centered medical home model of care and intensive care management in order to develop and contribute to a unique and sustainable approach to population health.
Activities
- 1 day a week working with the Camden Coalition of Healthcare Providers in Camden, NJ
- 2 days a week of clinical time seeing patients in an outpatient primary care setting
- 1 1/2 days a week working with the Crozer complex health team at Crozer Family Medicine
- 1/2 day a week for research and scholarly activity, case conferences and presentations
Personal Qualities and Attributes of Potential Fellows
- Graduate of an accredited Family Medicine Residency Program or Primary Care NP Program
- Excellent academic and clinical record
- Interest in care for underserved populations
- Interest in developing skills in hot-spotting, healthcare policy, medical home models, clinical redesign, trauma-informed care approaches, and intensive care management
- Proven ability to work in a diverse, team-based environment with healthcare and non-healthcare professionals
- Interest in trauma-informed care and harm reduction principles
