The National Committee for Quality Assurance (NCQA) recognizes Crozer Health Medical Group’s primary care practices as Level III Patient-Centered Medical Homes (PCMH). This is the NCQA’s highest designation for Medical Home Practices.

To earn this certification, practices must undergo a rigorous set of standards set forth by the National Committee for Quality Assurance.

All Crozer Health Medical Group physicians practicing at a recognized PCMH practice are certified by the NCQA. This certification is an indication of the dedication and quality of care our physician practices provide to patients every day.

Studies have demonstrated that following the medical home model can improve patient access and help to reduce medical costs, such as unnecessary admissions and emergency department visits.

What is a Patient-Centered Medical Home?

PCMH is a model of care designed to ensure you receive the right care, in the right amount, at the right time. In this model, your treatment plan is coordinated through your primary care provider who will clearly explain your treatment and help you to find any necessary specialists.

In a Patient-Centered Medical Home practice, patients can expect the following:

  • An ongoing relationship with a personal physician who works with a team at a specific location to take collective responsibility for their care
  • Care that is coordinated and integrated across the health care system
  • Care that follows the latest guidelines for quality and patient safety, including evidence-based medicine
  • Enhanced access to care through open scheduling, expanded hours and other options for communication between patients, physicians and practice staff
  • A practice that uses the latest information technologies to prescribe, communicate, track test results, obtain clinical information and monitor performance
